Things To Know Before Filling Out The Forms

There are two forms -- to download them, click on the links (names of forms):

• Registration Form/Payment Sheet
  The registration form/payment sheet should be filled out and submited by individuals only, not by a group of people. This form should be used by individuals who are registering to be new members of Duk County Association USA.

  This form can also be used by an individual who wants to submit his/her payment directly to our Treasurer instead of sending the payment through State Representative. Those who are paying their monthly or yearly member's fee are not required to pay $50.00 registration fee if they already paid in the past.

• Form of Payment
  Form of payment must be used by State representatives or a group of people without a State Representative in their state.

Mailing Information:

All checks and money orders must payable to "Trinity Evangelical Luther Church" with "Duk County Association USA" in the Memo Line
